Salary details: Grade 8/9: £29,447 - £39,513
Contract Type: Permanent, 37 hours per week
The post requires 7 day working on a rota basis based at Doncaster Royal Infirmary to facilitate discharges from the acute hospital trust. It attracts an additional salary payment of 0.5% and current rotas require weekend working around 1 in 8 weeks. Hours are 8AM – 4PM on those days. Non-working days are taken within the week prior or post the weekend worked.

An exciting opportunity has arisen for a Social Worker to join our North Locality Team. We are based in Bentley Resource Centre, Bentley, Doncaster. We are a busy but supportive team and look forward to welcoming a new SW in our team..
The Social Worker will have experience in adult services. We are particularly looking for a Social Worker who can prove their ability to work with complex cases across Adult Social Care and preferably have completed their ASYE, however consideration will be given to workers who are not at that level yet and have had experience of final placements in Adult Social Care.
The postholder will need to be able to work autonomously using initiative to manage own complex caseload. The role will involve undertaking holistic, strengths based assessments with adults, carers and families to help them make informed choices and decisions, enabling them to clarify and express their needs and achieve their outcomes, manage risk and contribute to service planning.
You will provide written and verbal records and reports, which are concise, factual and informative based on analysis of evidence. You will use evidence based practice and a strengths based conversation to identify need and take appropriate action, which is person centred and outcome focused.
You will also play a key role in the development and implementation of the locality based community led support model using creative ways to support people to live the lives they choose, helping them to stay in control whilst improving their confidence, connecting them with their local communities and supporting them to find solutions to their problems.
